What is the GUIDE program?
The Guiding an Improved Dementia Experience (GUIDE) Model is a voluntary nationwide model that aims to support people with dementia and their unpaid caregivers. The 8-year model, created by the Centers for Medicare & Medicaid Services (CMS), launched as a pilot on July 1, 2024 and expanded nationwide on July 1, 2025.
The GUIDE Model focuses on comprehensive, coordinated dementia care and aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities. It will achieve these goals through a comprehensive package of care coordination and care management, caregiver education and support, and respite services.

What does the program cost?
There is no cost to caregivers of eligible Medicare patients to participate in the Ceresti Dementia Care Program.
How long is the program?
GUIDE is an 8-year model which began in 2024, but you have the option to discontinue your participation in the Ceresti Dementia Care Program at any time.
What is respite care?
Respite care is short-term care that provides relief for family caregivers. It can be used to give caregivers time to rest, run errands or travel, etc. Through GUIDE, caregivers may receive up to $2,500 of covered respite care per year.
How much time does the program take each day?
On average, caregivers spend a few minutes using their tablet each day. Communication with your personal Ceresti Care Navigator can range anywhere from a brief check in to a more in-depth conversation based on your wants and needs.
Is the program available in different languages?
The Ceresti Dementia Care Program is offered in English and Spanish.
Will this program help engage a loved one with cognitive impairment?
Yes. For loved one’s with dementia and other types of cognitive impairment, this program provides engagement modules that play from the Ceresti Caregiver app (on the tablet). These engagement modules include reminiscence and music therapy. This type of increased engagement and stimulation is associated with positive emotions and improved ability to perform activities of daily living.
How do I access the tablet-based program?
Ceresti sends an iPad-like tablet (i.e., the Ceresti tablet) and accessories (shipping box, power cord, tablet stand, educational binder, stylus) to your home. The Ceresti tablet is pre-loaded with the Ceresti app and Internet access is included. No WiFi is needed. The Ceresti tablet is configured to be usable only for the Ceresti program.
Do I need to return the Ceresti tablet?
Yes. If you received a tablet from Ceresti as part of your program you will be asked to return it when you complete your program. Ceresti will provide you with a postage pre-paid shipping box to make the return process easy.
