# Ceresti Care Navigation for the CMS GUIDE Program

## About the program

The GUIDE Model is an 8-year voluntary nationwide model created by the Centers for Medicare & Medicaid Services (CMS). It launched as a pilot on July 1, 2024 and expanded nationwide on July 1, 2025. GUIDE focuses on comprehensive, coordinated dementia care to improve quality of life for people with dementia, reduce strain on unpaid caregivers, and help people with dementia remain in their homes and communities longer.

Ceresti delivers this benefit through:

- A dedicated Ceresti Care Navigator (Caregiver Coach) for ongoing 1:1 support
- Access to virtual clinicians proficient in dementia care
- A pre-loaded tablet with included internet — no Wi-Fi or technical experience required
- Daily educational content, caregiving tools, and assessments at the caregiver's own pace
- Engagement modules for the person with dementia, including reminiscence and music therapy
- Up to $2,500 per year of covered respite care (short-term care so caregivers can rest, run errands, or travel) for eligible caregivers
- Program available in English and Spanish

## Eligibility and cost

- Available to family caregivers of patients enrolled in Traditional Medicare (fee-for-service)
- No out-of-pocket cost to caregivers or patients — fully covered by Medicare
- Conditions supported: Alzheimer's disease and other dementias
- Caregivers may discontinue participation at any time
- The Ceresti tablet is loaned to participants and returned (postage pre-paid) when the program ends
