# Ceresti Care Navigation for the CMS GUIDE Program > guide.ceresti.com is the caregiver enrollment landing page for Ceresti Health's participation in the CMS GUIDE (Guiding an Improved Dementia Experience) Model — a Traditional Medicare benefit for family caregivers of people living with Alzheimer's disease and other dementias. The program provides 1:1 support from a dedicated Caregiver Coach, virtual access to dementia-proficient clinicians, personalized education delivered on a pre-configured Ceresti tablet (no Wi-Fi or technical experience needed), and up to $2,500 per year in respite care for eligible caregivers — fully covered by Medicare with no out-of-pocket cost. ## About the program The GUIDE Model is an 8-year voluntary nationwide model created by the Centers for Medicare & Medicaid Services (CMS). It launched as a pilot on July 1, 2024 and expanded nationwide on July 1, 2025. GUIDE focuses on comprehensive, coordinated dementia care to improve quality of life for people with dementia, reduce strain on unpaid caregivers, and help people with dementia remain in their homes and communities longer.
